Irish Wheelchair Association is a reputed company based in Dublin, County Dublin. We are looking for talented professional from the field of School Leavers to join our team as Young Adult Support Worker (lead Role: Person-centred Planning).

We would like to invite applications for the following, 35 Hours, 12-month fixed term contract position;

Young Adult Support Worker
(Lead Role: Person-Centred Planning)
Clontarf Balbriggan

As the Young Adult Support Worker with a Lead Role for Person-Centred Planning, you will support both the Rehabilitative Training and School Leaver Service Coordinators to deliver young adult day services with a strong person-centred focus in IWA’s Clontarf Community Centre, Balbriggan Community Hub and in service users’ communities. This will include facilitating personal goals work planning with young adult service users to ensure that each person is making regular progress towards their chosen goals. You will also support young adults (and staff members) to link with relevant external services and organisations, and to identify circles of support and resources for service users.


Liaison

Close liaison is required with the Rehabilitative Training and School

Leaver Service Coordinators and CSD Service Manager, as well as with other

staff working directly with young adults in Clontarf and Balbriggan.


Training, Experience and Qualifications

A minimum of 5 completed modules of

QQI level 5 in HealthCare Support or similar qualification

is essential. QQI Major Awards

that are deemed relevant to IWA are:


Community Health Services
(to include minor components

Safety Health at Work and Care Provision Practice)

Health Service Skills (to include minor components, Workplace Statutory Policies

Procedures, Care Skills and Care Support)

Health Care Support (to include minor components, Safety Health at Work, Care

Skills and Care Support)

Youth Work (no specific minor components required)


Knowledge and Skills

Previous experience of supporting young people with disabilities with person goals work is essential

Previous experience of developing and delivering projects or programmes is desirable

Good record-keeping and organisational skills

Knowledge of HSE’s New Directions Interim Standards and Person-Centred Planning Framework is an advantage

Previous experience of driving on IWA transport runs and knowledge of health safety is important

Salary range for this position is €26,762 to €30,686 DOE
