About the Role
This position is responsible for providing a clinical pharmacy service to the Transit Care Hub (TCH) and Patient Flow Unit. The pharmacist will facilitate discharge medication for patients that are being discharged from an inpatient ward through the TCH. The pharmacist will perform medication history, reconciliation and review for patients awaiting admission to a ward from the TCH.
