Sherwood Forest Hospitals NHS Foundation Trust is a reputed company based in Sutton-In-Ashfield. We are looking for talented professional from the field of Administration / Secretarial to join our team as Clinic Prep Clerk.

Within the Patient Services Department we support clinicians by ensuring that they have the relevant details of a patients medical history in order to provide safe and appropriate medical care. We ensure all of the following services are provided:
  • Pulling of medical records form file and refiling after use
  • Filing within case notes
  • Obtaining medical record folders and preparing them for use by clinicians when consulted by patients.
  • Locate and track notes
Main duties included are:
  • Locate the referral letter from the appropriate file or via Central Support in the case of target referrals if not already received from them.
  • Ensure sufficient history sheets available for doctor to write on and date the page ready for clinic attendance
  • Provide up to date front sheet and sufficient ID labels
  • Provide reconciliation slip appropriately completed with 18 week pathway details
  • Check all notes present and place in order of expected attendance.
  • Ensure notes are delivered to the appropriate clinic in time for the commencement of clinic
  • Ensure the patient details on all documents match those on the case notes – check the District number which is a unique identifier.
